When Anna had Sandersons baby, Maria Poole, the lady that was there during the delivery, was ashamed of Anna and spread the word that she was an unmarried mother, just before Annas baby passed away. Martha Perkins is another character that likes to spread gossip about people. This aspect alone is something everyone who cares to do well on the test should be prepared for. Question Format Originally, the ETS claimed that the test was designed not to require preparationafter all, reasoning is reasoning, right? Data Collection Plan Essay Abstract Postoperative Pulmonary complications (PPC) such as atelectasis, impaired gas exchange, pneumonia and acute respiratory failure (ARF) are not uncommon in patients undergoing major surgery and are a leading cause of increased patient morbidity, mortality and increased length of hospital stay (Yoder, Sharma, Hollingsworth, Talavera, Rice, 2013). Healthcare professionals are now expected to give patients verifiably effective treatments by implementing current evidence based practice (EBP). In order to do so, interventions to prevent PPCs must be investigated. The aim of this study was to evaluate the effectiveness of adding incentive spirometry (IS) to traditional postoperative chest physiotherapy (CPT) and respiratory care. Eighty patients between 30 and 50 year old who had undergone surgery participated in the study. Patients were divided into two groups: the intervention group where IS was used in addition to the traditional CPT, and the control group which included only traditional CPT. The results obtained in the investigation indicated that there were significant differences between both groups at the end of the study in terms of PPCs and hospital length of stay (LOS). Conclusion: Addition of IS to postoperative care helped in controlling PPCs. Hypothesis Because the risk for developing PPCs is not well studied specific numbers of occurrences are difficult to predict. However, it was hypothesized that patients who incorporated IS as part of their postoperative care would have a clinically significant decrease in the occurrence of PPCs and hospital LOS compared to those who did not incorporate IS into their postoperative care. Data Collection A quasi-experimental control trial was conducted from February 1, 2014 to May 31, 2014. The lead researcher, Adrienne Hinson, collected the data personally. During the four month study, eighty patients ranging in age from 30 to 50 years old who were undergoing surgery where the abdomen or chest was opened were selected randomly from the General Surgery Department at McLeod Regional Medical Center in Florence, SC. On sampling days, patients meeting the criteria were numbered chronologically based on their surgery time. For eighty consecutive business days, one of the patients was chosen randomly using a random numbers table created on Microsoft Excel. Patients who had recently been diagnosed with or treated for acute pulmonary complications, those who could not be instructed or supervised to assure appropriate use of the IS, and patients whose cooperation or comprehension was lacking were excluded from the study. All appropriate procedures were used to safeguard the rights of study participants. The study was externally reviewed by the hospital’s institutional review board before data collection began. Patients received verbal and written explanations of the study purpose and data collection procedures and after indicating willingness to participate in the study, voluntary informed consent was obtained (See Appendix A). Information included within the informed consent included participant status; participants were told which postoperative activities were routine, and which activities were being evaluated. Participants were informed that the data collected about them would be used for research purposes. The overall goals of the research, to evaluate to addition of IS to postoperative care, was described to patients. Participants were informed about which types of data would be collected, what procedures they would undergo, and how they were selected. The time frame of the commitment was defined as beginning on post-op day one and lasting until their follow up  visit after discharge. Information regarding funding was available to participants. All participants were educated on potential risks and benefits of participation. Finally, patients were assured that their privacy would be protected at all times and that they would be allowed to withdraw from the study at any point during their hospital stay (Polit Beck, 2012). The patients were deliberately placed into one of two experimental groups. Deliberate placement reduces confounding by ensuring the two groups were comparable when age, sex, smoking history, type of surgery and post-operative analgesia were compared. The control group consisted of 40 patients who underwent routine chest physiotherapy (CPT) postoperatively, as recommended by the Clinical Practice Guidelines set forth by The Agency for Healthcare Research Quality (2011). A physiotherapist supervised and assisted the treatment twice a day in the first two postoperative days and once a day from the third to the tenth days. During any session, the patients performed three to five deep breaths interspersed with periods of quiet breathing followed by two or three coughs or huffs with support from a pillow at least 10 times over a 15 minutes period. Additional techniques such as positioning and chest wall percussion were applied if breathing and coughing exercises alone were not effective in clearing excessive or retained pulmonary secretions. Patients were instructed to perform coughing and deep breathing exercises independently every hour while awake. The treatment group consisted of the remaining 40 patients who also followed the same Clinical Practice Guidelines but also received breathing exercise training with IS (Voldyne Volumetric manufactured by Sherwood Medical Company U.S.A.) In addition to the routine chest physiotherapy up to the tenth postoperative day. Application of breathing training with incentive spirometry was applied for five minutes, five times a day (El-Marakby et al., 2013). Monitoring began immediately postoperatively and continued until discharge. Hospital LOS was documented for each patient, along with whether or not the patient developed a PPC within 10 days of surgery. The researcher rounded on each hospitalized patient daily to ensure compliance and to update discharges and PPC diagnoses. In order to determine LOS, participant observation was used as a tool to collect data. LOS was calculated by subtracting day of admission from day of discharge. Date of admission was the date of surgery for all participants. Date of discharge was determined  by the surgical teams, who were blinded to the study and therefore measures are not likely to be influenced by reactivity (Polit Beck, 2012). PPCs were defined as any pulmonary abnormality that produced identifiable disease or dysfunction, was clinically significant, and adversely affected the postoperative clinical course (Hayden, Mayer, Stoller, 1995). Patients were monitored daily for 10 days for the diagnosis of a PPC. Given this definition, PPCs documented included atelectasis, pneumonia, acute respiratory failure, and exacerbation of COPD. Along with physical assessment and past history, biophysiological measures were used as data collection tools to aid in the diagnosis of PPCs (See Appendix C). Equipment and lab analysis were readily available and tests for biophysiological measures were completed by the appropriate staff at McLeod Regional Medical Center. Ordering tests and the interpretation of results was fulfilled solely by the healthcare providers as deemed necessary. The researchers were not responsible for diagnosing PPCs, but rather the observation and recording of data. Biophysiological measures for diagnosing acute respiratory failure include a low level of oxygen; a pulse oximetry falling below 80% saturation on room air or when arterial blood gas shows PaO2.

Traffic crashes are the leading cause of death for teens and young adults. More than 5,000 young people die every year in car crashes and thousands more are injured. Outline and Evaluate One Theory of the Formation of Romantic Relationships (4+8 Marks) Essay

The reward/ need satisfaction theory (RNS) was devised by Byron & Clore (1970) to explain the formation of romantic relationships, based on the principles of behavioral psychology. According to the theory, people form relationships with those who are most rewarding/ satisfying to be with which happens through conditioning. The elements of Skinner’s operant conditioning proposes that we repeat behaviors with positive outcomes (rewards) and avoid those with negative outcomes (punishments). Relationships positively reinforce by our partner satisfying our needs/rewarding us (through love or attention), but negative reinforcement also plays a part in the likelihood of formation as a relationship avoid us feeling lonely which both result in us seeking further contact with them thus forming a relationship. The theory also suggests that we may associate a person with positive feelings due to the even in which they meet; this is called classical conditioning. This form of conditioning involves pairing a neutral stimulus with an unconditioned stimulus to create a conditioned response, for example being at a party (NS) and feeling happy (UCR), then meeting a person (UCS) and associating this stimuli (now CS)with the happy feelings to create a CR of happiness whenever in their presence and we want to be with the person that makes us most happy, thus forming a relationship. An issue raised surrounding the RNS theory is that it fails to consider free will. It suggests that without rewards a relationship will not form however evidence from modern relationships argues otherwise, for example a rewards and satisfaction is given in one night stands, yet no relationship is formed, plus no association can be made through cyber relationships because the couple have not met, yet a relationship still manages to form. This evidence goes against the theory suggesting that other factors such as similarity, opportunity and our own choices play a role in relationship formation thus proposing the theory is deterministic as well as simplistic. On the contrary, research evidence has supported the RNS, one study conducted by Aron et al (2005). To begin the experiment, participants answered a questionnaire rating how intensely in love they were then they were shown photos of their partner during an MRI scan. It was found that dophamine-rich areas of the brain (area associated with rewards) had higher activity when shown their partner than when shown an image of their friend. These findings show that we form relationships with those who are rewarding. High temporal validity is a strength to this study because it was conducted recently. This means the couples used in the experiment have a higher ability to reflect modern day relationships therefore the findings are more generalisable to modern relationships. Along with that the study is strong physical, empirical evidence because of the use of MRI scans. Brain scans produce reliable, physical, solid evidence therefore have scientific proof for the link between relationships and rewards. The importance of reward level in determining relationship satisfaction was also demonstrated by Carte et al (1982) in which he asked 337 participants to asses their current relationship in terms of reward level and satisfaction and found that reward level was superior to all other factors in determining relationship satisfaction concluding that rewards are a greater factor than any when it comes to relationship formation. However a criticism of these findings is that the reward/need satisfaction theory only explores the receiving of rewards, whereas Hays (1985) found that we gain satisfaction from giving as well as receiving. Most of the research into RNS theory has been conducted in the US with US participants, making it difficult to generalize to different cultures. The theory, as well as the research, has an ethnocentric bias in the fact that it is based on relationships in western cultures, therefore reflect relationships of western societies, but these are very different to other cultures were people may not get a choice in their partner e.g. arranged marriages. Lott (1994) found that women in other cultures focused more on the needs of others than rewards, suggesting that there are differences in the value of rewards which this theory emphasizes so much about. As a result of this cultural bias, the RNS theory is not a universal explanation of relationship formation.

It can be acknowledged as an opportunity of KDC because the materials in the internal market are always available and it just has to deal with negligible carrying cost. Whereas, the wider spread of supplier network over the world market enables KDC to be flexible in ensuring the inputs for production.Along with the opportunities, suppliers also pose a lot of threats to the company in different ways, the first of which is through prices. As announced in the annual report of KDC in 2011, due to the inflation, the firm suffers from dramatically increasing and unstable prices of material together with the rises in salary and utilities expense, which leads to a significant drop in total revenue of KDC. Moreover, as KDC imports some materials from the external market, they are the price takers. Therefore, they will encounter some problems because of the fluctuations in the world prices.Back to the domestic market, the changing prices of inputs, especially sugar is really a pending issue. Not only KDC but also many confectionery producing corporations also get exhausted with the discrepancy between the sugar price at the beginni ng and the end of the harvest. While the price of confectionery does not change by 10 % per year, the sugar price fluctuates by nearly 30% (Hung, cited in thesaigontimes. vn, 2012) What is more, company effectiveness of performance is reduced significantly by supply chain disruption especially in global sourcing.Rob Handfield (2011, p3) points out that the delay in supplying can cut down shareholder value by as much as 10% or even worse in ‘time-sensitive† environments where early market introduction is critical to success. KDC also gets part of its inputs from the external market through shipping so it is impossible to avoid the risk of supply chain disruption, which means a great deal of time and money wasted. 3. 10 Pressure groups. One more factor that a firm often has to bear in mind derives from pressure groups, which are the ones attempting to influence the actions of organization.Every organization has its own pressure groups and as for KDC, the 2 groups that have the role in regulating KDC’s performances are Environmental Protection Group and Customer Right Protection Group. The EPG takes investigation of the treatment to wastes from production activities of KDC to determine whether it does harm to environment or not. If there is any problem, they will reflect it through the medium to force the company to correct it or make some compensation. The CRPG monitors the action of KDC from the step of taking inputs to processing and packaging to ensure it is safe to the health of customers.As the social attitudes change so does the power of pressure groups. In 2008, KDC had been blamed for using milk powder that contains melamine- one toxic substance- to produce cakes. Recommendations In term of specific environment, as confectionery demand is rather elastic due to its e normous number of close substitutes, the lower the price is, the higher the demand is, especially in these dark days of the economy.Hence, it is obvious that KDC should take a set of actions in terms of price to motivate consumers to purchase more such as reducing the price of products, or giving promotions when they buy a certain large amount. Such programs will not only raise the company’s revenue but also make its reputation much more well-known. The marketing work also should be taken into consideration since the current advertising campaign of KDC is not of high effectiveness. Thus to make the quality of its products known and then highly regarded by people, it should promote advertising and marketing strategy.As regard competition, to be more competitive to best exploit the market, it is thought that KDC should research and build development plans, diversifying its products and expanding to other markets, this will both helps KDC to avoid the case of lower revenue due t o the change of consumers’ taste and bring about enormously larger revenue if successful. For the suppliers, the careful analysis over opportunities and threats that supplier chains bring about gives the implications for the company to direct its actions. As for KDC, to minimize the negative impacts of suppliers, it has to face 2 ill-minded questions.The first one is how to reduce the purchasing prices of inputs. To enjoy the low-cost sourcing, it is advised that KDC should make a periodical purchase with extreme amount of materials. By that way, KDC is benefit from the great negotiation power over its partners about the prices. Another question is how to manage the risks from tardy delivery progress. According to Handfield, the most critical action is to develop the supplies chain strategies to detect the problems, assess the negative impacts and also mitigate them.In the case of KDC, they should sign the strict contracts with its partners, forcing them to adhere to the cont racts to ensure about the delivery progress. Moreover, it is better for KDC to make plans of frequently checking the materials left to be active in ensuring the inputs for manufacturing. Besides suppliers, KDC should pay attention to the pressure groups. The only way to get out of the impacts of them is doing things right. In particular, KDC is encouraged to give priority to the customer rights by providing the safe goods to consumers with appropriate prices.Moreover, it is necessary to develop a business plan of manufacturing parallel with protecting environment. It is the way KDC performs its social responsibility. Turning to external forces in the general environment, there is no denying that KDC should take these components into thorough consideration. Concerning opportunities, the firm should understand and take advantage of them to reinforce its competitive advantage over other competitors. For instance, in terms of demography, Vietnam seems to be a dynamic and potential marke t for confectionary products.Therefore, KDC should seek the ways to expand their scope of production as well as diversifying their items to meet new requirements from different customers. Turning to the issues that are generated by the general environment, the firm should stay alert and pay more attention to them if it does not want to be put under serious losses. Specifically, it is obvious that KDC should take into account the fluctuations in the open market as well as changes in the Government rules and regulations to respond quickly and make appropriate adjustments for its strategic management.IV. Conclusion To sum up, it is concluded with no doubt that KDC is doing an extremely good business, which brings about a strikingly high economic profit and also gets the KD’s brand name familiar with the customers in the domestic market as well as the global one. To attain this achievement, beside some internal factors of the company such as the good manager, skillful employees o r great capital, the external environment of KDC also makes a significant contribution.The general factor, which include economic, demographic, technological, social, cultural, political/legal, global, though do not have the direct impacts on the performance of KDC, they still serve as the frame to set the standard for all business activities. On the contrary, KDC is influenced by four specific elements namely customers, suppliers, competitors and pressure groups. They act as the determinants of the success or failure of KDC.
